Physician Orders for Life-Sustaining Treatment (POLST)
Legal document that states the type of medical care a person would like to receive during a medical emergency if they lack the capacity to do so.
Part of an Advance Directive, it must be signed by both physicians and patients and is usually provided by the patient's healthcare provider.
