Advance Health Care Directive

A series of legal documents that state the type of medical care a person would like to receive, or who will speak on their behalf, if they lack the capacity to do so during a medical emergency.

Typically, these types of documents are created by people who may be terminally ill or nearing end of life.

It includes organ donation preferences, Do Not Resuscitate (DNR) Orders, Physician Orders for Life-Sustaining Treatment (POLSDT) , and Durable Power of Attorney.

Also known as a Living Will.
